No problem! Below are downloadable versions of all current CKC registration applications and other forms. Simply download, complete, and submit it to CKC by mail, fax, or email.
Registration Applications
Canine Registration Application
Use this application to apply for official CKC registration of an adult canine for which the sire and dam are registered with CKC or another CKC-recognized breed registry, or to apply for registration of a canine which is already registered with a CKC-recognized registry.
Litter Registration Application
Use this application to apply for CKC registration of a puppy litter and request Pre-Printed Puppy Applications for each puppy in the litter.
If the breeder of the litter owns both the sire and dam, then both parent dogs must be registered with Continental Kennel Club in order for the litter to be registered.
PAW Registration Application
Use this application if you'd like to apply for evaluation for a dog with no prior registration or for a dog whose paperwork has been lost, stolen, or damaged.
CKC charges a NON-REFUNDABLE evaluation fee of $50 per application. Submitting this application does not guarantee registration.
Non-Purebred Canine Application
Complete and submit a Non-Purebred Canine Registration Application.
You will need two witnesses at least 18 years of age to attest to the accuracy of the information provided and 3 photographs of the applicant's canine.
Kennel Registration Application
This application is used by breeders and/or kennel members to apply for CKC registration of adult canine(s) qualifying for registration with Continental Kennel Club Inc. Apply for CKC registration of up to ten (10) dogs on this application.
Additional Forms
Co-Ownership Authorization Form
Use this form to request the listing of a co-owner on the official CKC Registration Certificate for an applicant canine submitted for registration with CKC.
Change of Address Form
Use this form to report a change in your mailing address. It may be submitted via mail, email, or fax. Once received, your mailing address will be updated on your account.
Supplemental Transfer Form
Use this form if you would like to transfer ownership of a dog to a new owner and the original Certificate of Registration has been misplaced.
Certification of Ownership
Use this form as supporting documentation for ownership. It may accompany a transfer of ownership in which the previous owner failed to sign the transfer or, in some instances, dual registration with CKC.
